The survey facility is available for use by faculty/staff to enable the collection of data relating to the business or academic needs of the campus.
The collection of private information is specifically prohibited unless authorized by the Information Security Team.
Requests for authorization may be sent to Lyle Gomes, Chair of the Information Security Team.
In addition, if data is being collected for research purposes, approval of the Committee on Research Involving Human Subjects may be necessary.
